分析颈动脉海绵窦瘘CT、MR、DSA的诊断研究对比

目的分析颈动脉海绵窦瘘的CT、MR、DSA诊断价值。方法回顾性分析经血管内栓塞治疗成功的颈动脉海绵窦瘘的11例患者的诊治资料。对CT、MR、DSA诊断结果做探讨分析。结果 CT、MR检查表现:患侧眼球突出、眼上静脉充盈扩张、海绵窦扩大;有患者临床表现为眼球壁增厚,眼外肌扩张,岩上下窦扩张,眼下静脉、内眦经脉。DSA表现则是除可见患侧眼上静脉扩张、海绵窦扩大外,还可见瘘口、引流静脉、盗血等。结论 CT、MR、DSA在诊断CFF疾病方面各有优势,DSA能够检查CFF患者血流动力学改变情况,而CT、MR诊断则不能提示这一结果。

Analysis on Diagnosis Study of CT, MR and DSA for Carotid-cavernous Fistula

Objeetive To analyze diagnostic value of CT, MR and DSA for carotid-cavernous fistula. Methods The clinical data of 11 patients with carotid- cavernous fistula treated successfully with endovascular embolization were analyzed retrospectively. And diagnostic results of CT, MR and DSA were observed and analyzed. Results The examination of CT and MR shows that proptosis, filling and stretching of superior ophthalmic vein, and expansion of cavernous sinus. The clinical manifestation is thicken of eyeball wall, expansion of extrinsic eyeball muscles, expansion of superior and inferior petrosal sinus, inferior ophthalmic vein and inner canthus meridians. Representation of DSA are fistula, draining vein and steal phenomena besides expansion of superior ophthalmic vein, and amplification of cavernous sinus, Conclusion CT, MR and DSA have different advantages in diagnosing CFF. DSA can examine the changes of haemodynamics for patients with CFF, but CT and MR can＇t prompt the result.
