鼻咽癌放疗中顺铂增敏作用和紫杉醇增敏作用的比较

目的探讨顺铂和紫杉醇在鼻咽癌放射治疗中的增敏作用和不良反应。方法选取2005年6月～2010年6月于湖南省郴州市第一人民医院肿瘤科首次治疗的102例局部晚期鼻咽癌患者，随机分顺铂增敏组（C组，n=50）与紫杉醇增敏组（P组，n=52）。用药方法：C组顺铂每周用药1次，每次按40mg／m^2剂量用药，用药后1h内进行放疗，到放疗结束；P组按紫杉醇60mg，m^2药量静脉滴注，每周1次，共用7周。放疗方法：先定制面模固定，采用常规放疗方法。结果治疗后，两组在原发灶及颈部淋巴结退缩方面的差异没有统计学意义（P〉0．05）。C组（52．0％）转移率高于P组（28．8％），且差异有统计学意义（P〈0．05），而两组在复发率和生存率方面差异无统计学意义（P〉0．05）。P组在2、3级口腔黏膜炎、白细胞减少方面较C组明显，并且差异有高度统计学意义（P〈0．01）。结论顺铂与紫杉醇在鼻咽癌放射治疗中的增敏作用相似，但顺铂毒副作用相对较小。

Radio-sensitization effect of cisplatin versus paclitaxel for local advanced nasopharyngeal carcinoma

Objective To evaluate the radio-sensitization effect and side effect of Cisplatin versus Paelitaxel in local advanced nasopharyngeal carcinoma. Methods 102 patients were randomly divided into the group C （n = 50） and the group P （n = 52）. The group C was used venous with 40 mg/m^2 Cisplatin once a week. The P group was received Paclitaxel by 60 mg/m^2 via intravenous infusion, every 7 days for 7 times in the radiotherapy duration. All patients were received conventional radiotherapy. Results Complete tumor remission, regional recurrence rate and survive rate were no significant difference between the two groups. The metastasis rate in the group C （52.0%） was significantly higher than in the group P （28.8%） （P 〈 0.05）. Oral cavity mucositis in grade 2 and 3, leukopenia in the group P were more fre- quent than in the group C （P 〈 0.01）. Conclusion Cisplatin is similar to Paditaxel in radio-sensitization efficiency for local advanced nasopharyngeal carcinoma, but the side effect of Paelitaxel is more severe than Cisplatin.
