应用PPH器械完成超低位直肠癌保肛术研究

目的探讨应用一次性肛肠吻合器（PPH）完成超低位直肠癌结肠肛管吻合术的效果。方法选择2011年5月～2012年6月涿州市医院治疗的超低位直肠癌患者69例为研究对象,采用PPH器械完成结肠肛管吻合术,观察手术效果及并发症情况。结果 69例患者顺利完成超低位保肛手术,术后吻合口漏2例（2.9%）,吻合口狭窄1例（1.5%）,4例因排便功能障碍行乙状结肠造口术（5.8%）。1例因局部复发行Miles手术（1.5%）。结论应用PPH器械完成直肠癌超低位吻合安全有效,经济实用,尤其是对肥胖患者、骨盆狭小的男性患者大大提高了保肛几率,适宜在广大医院推广。

Study on the application of PPH appliances in anus-preserving operation for ultra-low rectal carcinoma

Objective To analyze the application effect of PPH appliances in anus-preserving operation for ultralowrectal carcinoma.Methods 69 patients with ultra-low rectal carcinoma from May 2011 to June 2012 in Hospital of Zhuozhou City were selected and treated with colon anus anastomosis by PPH appliances.The operation effect and complications situation were observed.Results The anus-preserving operation for ultra-low rectal carcinoma of 69 patients were successfully completed,2 cases（2.9%） of anastomotic leakage were found after the operation,1 case（1.5%） of anastomotic stenosis was found,4 cases（5.8%） were treated with sigmoid colostomy because of the defecation dysfunction.And 1 case（1.5%） was treated with Miles operation because of the local recurrence.Conclusion The PPH appliances in anus-preserving operation for ultra-low rectal carcinoma is effective and safe,and it is economical and practical.The operation improves the anus-preserving probability especially for the obese patients and male patients with pelvic narrow,it is suitable for extension in majority hospitals.
