高龄多器官功能障碍患者首次经外周静脉留置中心静脉导管置管接受度及影响因素

目的了解高龄多器官功能障碍患者首次对经外周静脉留置中心静脉导管（PICC）置管的接受情况，分析相关影响因素，促进PICC置管在高龄多器官功能障碍患者中的应用。方法参考文献。设计调查问卷，对182例高龄多器官功能障碍患者进行PICC置管问卷调查，内容包括患者对PICC的接受情况、年龄、穿刺技术、穿刺疼痛、并发症的发生、治疗态度、影响日常生活和院外导管维护等多个因素对患者的影响程度。结果接受PICC置管患者116例，未接受PICC置管患者66例。穿刺技术、并发症的发生、治疗态度、病程分段及院外导管维护5个因素人Logistic多元回归方程。66例患者不接受PICC的主要原因是影响日常生活、不易维护、并发症和治疗态度。结论提高PICC置管技术，进行科学的宣传，消除患者疑虑，在高龄多器官功能障碍患者中推广PICC，让更多高龄多器官功能障碍患者受益于PICC。

Multiple factor investigation and logistic analysis of the reception of PICC insertion in elderly multi-organ failure patients

Objective To explore the receptional condition of peripherally inserted central catheter （PICC） placement in very elderly multi-organ insufficiency patients and analyze the correlated influence factors in order to improve the application of PICC in very elderly patients. Methods PICC correlated knowledge was explained to 182 very elderly multi-organ insufficiency patients. Questionnaire designed by ourselves was used to invest the influence factors which including reception to PICC, age, puncture technique, puncture pain, complication, treatment attitude, influence on daily life and catheter handling out of hospital. Results 116 patients accepted PICC and 66 patients refused PICC. Puncture technique, complication, treatment attitude, influence on daily life and catheter handling out of hospital were analyzed by logistic regression equation. The main influence factors were complication, treatment attitude, influence on daily life and catheter handling out of hospital. Conclusion Improving PICC implanting technique, carrying out scientific propaganda, eliminating patient＇s misgivings and promoting PICC in very elderly multi-organ insufficiency patients can make more patients profit from PICC.
