摘    要:目的:提高对多发性硬化(MS)的临床与MRI表现的认识。方法:回顾性分析50例临床确诊的MS的临床与MRI表现及临床定位与MRI定位的关系。结果:50例MS在MRI上均发现病灶,MRI可显示临床上无定位体征的大脑半球上的亚临床病灶,对临床上有定位体征的脑干、小脑、脊髓的病灶可全部显示,对视神经病变显示欠佳。MS斑块在T2WI均为高信号,在T1WI为等或低信号,急性期病灶周边明显强化,激素治疗后病灶明显减少并缩小。结论:MRI对显示视神经以外的MS病灶高度敏感,并可显示临床上无定位体征的亚临床病灶,为MS临床诊断提供可靠依据,可作为确诊MS的首选检查方法。

Abstract:Purpose:To increase the ability to recognize the clinical and MRI features of multiple sclerosis.Methods:50 cases of MS clinical and MRI manifestations was analyzed,meanwhile the clinical and MRI location was studied retrospectively.Results:All of the patients in 50 cases were found MS lesions in MRI,and MRI can detect the lesions which was asymptomatic MS plaques in cerebral.MRI can find all of the lesions in brainstem,cerebellum,spinal which was found by clinical examination,but in optic nerve was not so good.MS Plaques appears high signal intensity in T 2WI and low or middle signal intensity in T 1WI. The plaques in acute or active stage were enhanced by Gd DTPA.The plaques were reduced after treated by corticosteroid.Conclusion:MRI was found sensitivity in detecting MS plaques,include asymptomatic lesions but in optic nerve.It should be the first examination to be performed in the diagnostic protocol of MS.
