丹参冻干粉治疗慢性阻塞性肺疾病急性加重期的疗效观察

【目的】观察丹参冻干粉治疗慢性阻塞性肺疾病急性加重期（AECOPD）的疗效及不良反应。【方法】160例AECOPD患者随机分为治疗组和对照组，每组各80例。两组均行常规治疗，包括鼻导管吸氧1～2L／min、抗感染、解痉平喘、祛痰；治疗组在此基础上加用丹参冻干粉800mg加入0．9％氯化钠注射液250mL中静脉滴注，每日一次，疗程为10d。观察两组患者治疗前后血气分析、临床疗效、血液流变学的指标变化及不良反应。【结果】治疗后对照组血液流变学各项指标无明显改善（P〉0．05），治疗组全血黏度、血浆黏度、全血还原黏度和纤维蛋白原比治疗前显著降低，且差异有显著性（P〈0．01），两组治疗后各指标组间比较差异有非常显著性（P〈0．01）。两组治疗前血气分析指标无显著差异（P〉0．05），治疗后动脉血氧分压（Pa02）增加，二氧化碳分压（PaCO2）降低，与治疗前比较及治疗后组间比较差异均有显著性（P〈0．01）。治疗组总有效率为85％明显高于对照组（65％），且差异有显著性（P〈0．05）。两组治疗期间均未出现明显不良反应。【结论】在常规治疗基础上同时加用丹参冻干粉治疗AECOPD疗效佳，无明显不良反应。

Observation on the Efficacy of Danshen Freeze-dried Powder for the Treatment of Acute Exacer- bation of Chronic Obstructive Pulmonary Disease

Objective] To observe the efficacy and adverse reaction of Danshen freeze-dried powder dose for the treatment of acute exacerbation of chronic obstructive pulmonary disease（AECOPD）. Methods] Totally 160 AECOPD patients were randomly divided into treatment group and control group with 80 cases in each group. Both two groups received conventional treatment including 1-2 L/rain oxygen inhalation by nasal catheter, anti-infection, relief of spasm, antiasthma and eliminating phlegm. On the basis of conventional treatment, the treatment group was additionally treated with intravenous drip of 800rag Dandhen freeze dried powder added into 250rag sodium chloride injection once per day for 10 days. The blood gas analysis, clinical efficacy and the changes of hemorrheology indexes before and after the treatment as well as adverse reactions of two groups were observed. Results] The hemorrheology indexes in control group after treatment had no sig- nificant improvement （ P 〉0.05）. Compared with before treatment, whole blood viscosity, plasma viscosity, whole blood reduced viscosity and fibrinogen in treatment group after treatment decreased significantly, and there was significant differences（ P 〈0.01）. There were very significant differences in the indexes after treat- ment between two groups（ P 〈0. 01）. There was no significant difference in blood gas analysis indexes before treatment between two groups（ P 〉0.05）. After the treatment, PaCO2 reduced and PaO2 increased（ P 〈0.01）, and there were significant differences between before and after treatment or two groups after the treat- ment（ P 〈0. 01）. The total effectiveness of treatment group was 85%, which was obviously higher than that of control group（65 %）, and there was significant difference（ P 〈0.05）. There was no obvious adverse reac- tion in two groups during the treatment. Conclusion]Conventional therapy combined with Danshen freeze- dried powder for the treatment of AECOPD has good effect and no obvious adverse reactions.
