全电视胸腔镜与传统开胸手术行肺叶切除的比较研究

【目的】对比全胸腔镜下肺叶切除术与传统肺叶切除术患者的相关临床指标，探讨胸腔镜手术治疗可行性及应用价值。【方法】分析实行全胸腔镜下及传统肺叶切除术的肺癌患者共为176例，施行全胸腔镜下肺叶切除术95例，传统肺叶切除术81例，对比两组的手术时间、术中出血量、淋巴结清扫枚数、术后疼痛、胸腔引流时间、住院时间、手术费用、术后并发症、术后肿瘤转移复发等有无差异。【结果】VATS组手术时间、胸管引流时间较传统组差异无统计学意义（P〉0．05）；与传统组相比，VATS组术后疼痛明显减轻、住院时间减少、出血量明显减少；两组均无死亡。【结论】VATS行肺癌根治术具有微创、安全、恢复快、并发症少等优点，可以作为早期肺癌外科治疗的一种方案。’

Comparative Study of Complete Video-assisted Thoracoscopic Surgery vs Conventional Thoracic Surgery for Lobectomy of Lungs

Objective]To compare the clinical indicators of patients undergoing complete video-assisted thoracoscopic（VATS） lobectomy vs conventional lobectomy, and to explore the feasibility and application val- ue of thoracoscopic operation. Methods] Totally 176 patients with lung cancer undergoing complete VATS lobectomy（95 cases） or conventional lobectomy（81 eases）. The operation time, intraoperative blood loss, number of lymph node dissection, postoperative pain, chest drainage time, hospital day, surgery cost, postop- erative complication and postoperative recurrence and metastasis of tumor were compared between two groups. Results] There was no significant difference in the operation time and chest drainage time between two groups （P 〉0. 05）. Compared with conventional group, postoperative pain obviously decreased, hospital day de- creased and bleeding volume obviously reduced in VATS group. No death occurred in two groups. Conclusion] VATS lobectomy has advantages such as minimal trauma, safety, rapid recovery and less complication. Therefore, it can be used as a surgical method of early lung cancer.
