基于腰椎间盘突出应用MRI与CT诊断的临床特异性比较

目的:探究在腰椎间盘突出诊断中MRI与CT的特异性及准确度。方法:由本院2019年11月—2020年10月期间内就诊群体,随机选择60例患者作为对比研究对象,对60例患者分别通过MRI与CT进行检查,以手术病理结果作为诊断金标准,对比两种诊断手段对60例患者的结果准确性,同时对比两种诊断手段的征象。结果:60例患者均经手术病理诊断为腰椎间盘突出症,MRI确诊57例患者,诊断准确率为95%,CT确诊49例患者,诊断准确率为81.67%,组间比较有明显差异;CT对钙化、积气的征象诊出率明显高于MRI,MRI对椎间盘变性、硬膜囊受压、脊髓变性征象的检出率明显高于CT,两者在神经根受压的检出率不存在差异性。结论:相较CT,MRI对腰椎间盘突出的诊断准确性更高,但CT也有其应用价值,对钙化和积气的检出率更高,因此临床上可根据实际情况进行选择,必要时可联合应用。

Comparison of clinical specificity between MRI and CT in diagnosis of lumbar disc herniation

Objective To explore the specificity and accuracy of MRI and CT in the diagnosis of lumbar disc herniation.Methods In November 2019 in October 2020,during the period of medical groups,60 patients were randomly selected for comparison the research object,respectively in patients with 60 cases by MRI and CT inspection,with surgical pathology results as a diagnostic gold standard,compared to two kinds of means diagnosis accuracy,and the results of 60 patients at the same time comparing two sign of diagnostics.Results All the 60 patients were diagnosed as lumbar disc herniation by surgery and pathology,the diagnostic accuracy was 95%in 57 patients diagnosed by MRI,and 81.67%in 49 patients diagnosed by CT,indicating significant differences between the two groups.CT showed significantly higher detection rate of calcification and gas accumulation than MRI,MRI showed significantly higher detection rate of disc degeneration,dural sac compression and spinal degeneration than CT,and there was no difference in detection rate of nerve root compression between the two.Conclusion Compared with CT,MRI has a higher diagnostic accuracy for lumbar disc herniation,but CT also has its application value,with a higher detection rate of calcification and gas accumulation.Therefore,it can be selected clinically according to the actual situation,and combined application can be conducted when necessary.
