预注右美托咪定用于喉显微手术麻醉的临床效果

目的探讨预注右美托咪定用于喉显微手术麻醉的临床效果。方法在我院2018年1月至2019年12月期间诊治的喉显微手术患者中选取52例作为研究对象,并按随机抽签法分组(各26例),对照组麻醉诱导前不预注麻醉药,研究组预注0.6μg/kg右美托咪定。记录两组患者麻醉诱导前、插管即刻、置入支撑喉镜时、拔管后的应激反应指标变化,同时比较两组患者术后睁眼时间、苏醒时间、离开手术室时间、不良反应发生率差异。结果研究组患者在麻醉诱导前、拔管后5 min的平均动脉压、心率、肾上腺素以及去甲肾上腺素均低于对照组(P<0.05);研究组患者术后睁眼时间、苏醒时间、离开手术室时间与对照组相比,差异均无统计学意义(P>0.05);研究组患者不良反应发生率为7.69%,低于对照组的38.46%(P<0.05)。结论预注右美托咪定用于喉显微手术麻醉的临床效果良好,可有效缓解患者围术期应激反应,并促使患者术后快速睁眼、清醒,且安全性较高,适宜喉显微手术患者应用。

Clinical effect of pre-injection of dexmedetomidine in laryngeal microsurgery anesthesia

Objective To investigate the clinical effect of dexmedetomidine pre-injection in anesthesia of laryngeal microsurgery.Methods A total of 52 patients with laryngeal microsurgery diagnosed and treated in the hospital from January 2018 to December 2019 were selected as research subjects and grouped by random drawing(n=26).The control group was not pre-injected with anesthetics before anesthesia induction,while the study group was pre-injected with 0.6μg/kg dexmedetomidine.Changes in the stress response indexes before anesthesia induction,immediately after intubation,when the laryngoscope was placed,and after extubation were recorded in the two groups of patients.The differences in eye-opening time,wake-up time,time to leave the operating room,and incidence of adverse reactions were compared between the two groups.Results The mean arterial pressure,heart rate,epinephrine,and norepinephrine of patients in the study group before induction of anesthesia and 5 minutes after extubation were lower than those in the control group(P<0.05).Comparing the study group with the control group,there was no significant difference in the eye-opening time,wake-up time,or the time to leave the operating room(P>0.05).The incidence of adverse reactions in the study group was 7.69%,which was much lower than that of the control group(38.46%,P<0.05).Conclusion Pre-injection of dexmedetomidine for laryngeal microsurgery anesthesia has a good clinical effect,it can effectively alleviate the perioperative stress response of patients,and shorten the time to open eyes and gain consciousness,with high safety.It is suitable for patients receiving laryngeal microsurgery.
