十二指肠乳头喷洒利多卡因对ERCP术后胰腺炎发生和血淀粉酶水平的影响

目的探讨十二指肠乳头局部喷洒利多卡因对经内镜逆行胰胆管造影(endoscopic retrograde cholangiopan-creatography,ERCP)术后胰腺炎发生和血淀粉酶水平的影响。方法需要接受ERCP手术的112例患者随机分为利多卡因组和对照组,利多卡因组55例,对照组57例。前者在十二指肠乳头插管前给予10mL 1%利多卡因乳头局部喷洒,后者给予生理盐水,比较两组ERCP后胰腺炎和高淀粉酶血症的发生情况。结果利多卡因组术后胰腺炎发生率(3.64%vs 15.79%,P=0.031)和高淀粉酶血症发生率(18.18%vs 40.35%,P=0.01)均明显低于对照组。结论十二指肠乳头局部喷洒利多卡因可以降低ERCP术后胰腺炎和高淀粉酶血症的发生。

Effects of Lidocaine Sprayed on the Majou Duodenal Papilla on the Prevention of Post-ercp Pancreatitis and Hyperamylasemia

Objective To evaluate the preventive effects of lidocaine sprayed on the major duodenal papilla on the frequency of post-ERCP pancreatitis and hyperam-ylasemia.Methods A total of 112 patients were recruited.Patients undergoing ERCP were randomly divided into lidocaine group(55 patients) and control group(57 patients) who had 10 mL of 1% lidocaine or normal saline solution sprayed on the major papilla before cannulation.The end point was the incidence of postERCP pancreatitis and hyperamylasemia.Results There was no significant difference in basic information,such as age,gender and diagnosis indictions of ERCP between lidocaine group and control group.The frequencies of post-ERCP pancreatitis(3.64% vs 15.79%,P=0.031) and hyperamylasemia(18.18% vs 40.35%,P=0.01) in lidocaine group were both significantly lower than that in controls.Conclusion The results indicate that lidocaine sprayed on the major papilla can prevent post-ERCP pancreatitis and hyperamylasemia.
