4例剥脱性骨软骨炎的影像学表现

目的：加深对剥脱性骨软骨炎的影像学认识,提高影像学诊断水平.材料与方法：对4例经关节镜检查治疗病理证实的剥脱性骨软骨炎的影像学资料回顾性分析.结果：影像学表现为：病灶单发,都在距骨滑车,X线片上为距骨滑车一侧关节面软骨及与相连的软骨下骨质卵圆形骨质缺损区并见剥脱的小骨片,边缘硬化.CT图像上为圆形或卵圆形骨块,剥脱骨块长径约为数毫米,边缘透光带,透光带外围骨质发生硬化,邻近骨关节面伴有少量积液.结论：本病病因不十分明确,外伤病史有参考价值,病理改变主要为关节软骨及与相连的软骨下骨质局部无菌性坏死,并非真正的炎症.本病的确诊需关节镜及病理学检查.

Imaging performance of four cases of osteochondritis dissecans

Purpose： To deepen osteochondritis dissecans radiological awareness, improved imaging diagnosis. Materials and Methods：Four patients with pathologically confirmed arthroscopic treatment of osteochondritis dissecans of the imaging data were analyzed retrospectively. Results： Imaging performance： Single lesions are on the talus tackles, X-ray on one side of the articular surface of the talus trochlear cartilage and bone oval and subchondral bone defect linked and see a small piece of bone stripping edge hardened. CT images were round or oval bone, bone stripping diameter of about a few millimeters long, with translucent edges, translucent with peripheral bone sclerosis occurs close to the surface with a small amount of bone and joint effusion. Conclusion： The cause of the disease is not very clear, there is a history of trauma reference value, mainly pathological changes of articular cartilage and the subchondral bone is connected to the local aseptic necrosis, not really inflammation. Diagnosed with the disease need arthroscopic and pathological examination.
