抗环瓜氨酸肽抗体与RF对诊断老年类风湿关节炎的意义

①目的探讨抗环瓜氨酸肽抗体（抗CCP抗体）和类风湿因子（RF）在老年类风湿关节炎中的敏感度及特异度及二者联合检测在老年类风湿关节炙中的意义。②方法采用ELISA检测抗CCP抗体，用胶乳增强免疫透射比浊法定量检测RF。共检测19例老年类风湿关节炎患者、23倒非老年RA中的抗CCP抗体及RF的分布。比较二者之间的敏感度、特异度及二者联合检测的结果。③结果抗CCP抗体及RF对RA的敏感度分别为84．57％、78．29％，特异度分别为94．74％、73．68％，抗CCP抗体敏感度特异度与RF敏感度差异无显著性（P〉0．05），抗CCP抗体和RF比较差异有显著性（P〈0．05）。④结论抗CCP抗体与RF联合检测能提高诊断老年RA的敏感性和特异性。

SSignificance of anti cyclic citrullinated peptide antibody and RF in diagnosis of elderly rheumatoid arthritis

Objective Simultaneous detection of anti -cyclic citnallinated peptide antibodies （anti- CCP antibody） and rheumatoid factor （RF） in elderly patients with rheumatoid arthritis in the sensitivity and specificity, and to explore the combined detection of two elderly patients with rheumatoid arthritis. Methods ELISA detection of anti - CCP antibodies, using latex to en- hance the immune turbidimetric quantification of rheumatoid factor （RF）. Were detected 19 cases of elderly patients with rheumatoid arthritis, 23 cases of non - elderly RA in anti - CCP antibody and RF distribution. Results The results of anti - CCP antibody and RF sensitivity of the RA were84.57 % ,78. 29%, specificity were 94.74%, 73.68%. The sensitivity and specificity of anti - CCP antibody and RF was no significant differ- ence in sensitivity（ P 〉 0.05）, anti -CCP antibody and RF difference was significant（ P 〈 0.05）. Conclusion Anti- CCP anti- body has a good sensitivity and specificity, and RF joint detectiQn can improve the diagnostic sensitivity and specificity of the elderly RA.
