静脉动脉化治疗下肢血栓闭塞性脉管炎

对8例(9个下肢)血栓闭塞性脉管炎患者进行了静脉动脉化手术治疗,一段EPTFE人造血管或大隐静脉置于股总或股浅动脉与股浅静脉之间或置于腘动脉与腘静脉之间形成动静脉瘘,2个月后,将瘘口近端静脉留置线抽紧打结,从而使动脉血通过静脉单向灌注下肢远端的缺血组织。经过2～14个月,平均8.5个月的随访,全部病例均获满意疗效。本手术是治疗下肢严重缺血症的有效方法,EPTFE人造血管是一可靠、有效的血管替代材料。

Arterialization of Venous Channel in the Treatment of Thromboangiitis Obliterans of Lower Extremity

Thromboangiitis obliterans was treated with arterialization of venous channel in 8 cases (9 limbs). A short segment of EPTFE graft or the saphenous vein is to be interposed between the common or superficial femoral artery and the superficial femoral vein or between the popliteal artery and popliteal vein to form an arteriovenous fistula. Two months later, the prozimal venous limb of the fistula is tied to convert the fistula into direct arteriovenous reversal. The length of a follow-up study ranged from 2 to 14 months with an average of 8.5 months. In all patients the postoperative results are excellent or good. This operation is an effective method for the treatment of severely ischemic lower extremity and EPTFE prosthetic vascular graft is a reliable and effective vascular graft.
