鲍曼不动杆菌耐药谱4年动态观察与分析

目的：了解广州地区鲍曼不动杆菌检出情况及其对常用抗菌药物的耐药谱动态变化，为临床合理选用抗菌药物提供依据。方法：应用回顾性调查方法，对本院从2000年1月至2003年6月间临床样本分离的鲍曼不动杆菌的药敏试验进行对比统计分析。结果：所分离出的194株鲍曼不动杆菌，占革兰阴性菌的6．82％；鲍曼不动杆菌对常用抗菌药物的耐药率总体呈上升趋势。对头孢噻肟、头孢曲松的耐药率为41．7％和43．5％，中介率分别为45．8％和52．2％。耐药率低于30％的有阿米卡星和氧氟沙星。其次是加酶抑制剂抗菌药物氨苄西林．舒巴坦、头孢哌酮．舒巴坦，耐药率为32．6％～39．1％。对亚胺培南仍呈高度敏感，敏感率为100％。结论：本地区鲍曼不动杆菌检出率维持在较稳定的水平。动态监测鲍曼不动杆菌的耐药谱变化，是防止鲍曼不动杆菌的感染率攀升的重要措施。

Analysis on changes of antibiotic resistance spectrum of acinetobacter baumannii during past four years

AIM:To investigate the changes of antibiotic resistance spectrum of Acinetobacter baumannii(A. baumannii). METHODS: A. baumannii were isolated from clinical samples and their resistances to antibiotic in our hospital from Jan,2000 to Jun ,2003 were analysed and studied retrospectively. RESULTS: A total of 194 strains of A. baumannii were isolated. The checkout rates were 6.82% of G - bacteria. There was a elevated tendency of the resistance rates to common antibacterials in A. baumannii during the past 4a. The resistance rates of A. baumannii to amikacin, ofloxacin were less than 30%, to cefoperazone-sulbactam and ampicillin-sulbactam were 32.6%-39.1% and to cefotaxime and ceftriaxone were 41.7% and 43.5%, intervene rates were 45.8% and 52.2%.The A. baumannii, highly sensitive to imipenem. CONCLUSION: The rate of A. baumannii in our hospital was steadly maintained during past 4a. To prevent the A. baumannii infection outbreak,it is very important to advocate the reasonable medication and control the use of broad-spectrum antibiotic and carry out the drug sensitive trial of bacteria before beginning treatment of a infection by A. baumannii.
