摘    要:目的探讨MRI上骨肉瘤中骨膜异常与肿瘤特征的相关性。方法对17例骨肉瘤的影像与病理进行研究,观察骨膜异常形态,对骨膜异常、肿瘤髓内侵犯、骨皮质破坏范围及软组织肿块径线进行影像病理对照及相关性分析。结果 MRI可显示病理所见的各种骨膜异常,二者显示范围无统计学差异。MRI上,骨膜异常范围与骨髓内侵犯范围、骨皮质破坏范围的相关系数分别为0.901与0.764;骨膜纵向范围与肿瘤皮质外纵径和皮质外横径的相关系数分别为0.717和0.100。结论 MRI对骨膜异常范围的显示有较高的准确性。骨膜异常范围与骨髓内侵犯范围、骨皮质破坏范围及软组织肿块特性具有相关性。

Abstract:Objective To analyze the correlation between periosteal abnormality and tumor characteristics of osteosarcoma on MRI. Methods Seventeen osteosarcoma specimens were evaluated. The type and extent of periosteal abnormality, bone marrow invasion, cortical destruction and size of the soft tissue masses were compared radiologically and pathologically. Results All types of periosteal abnormality were demonstrated on MRI. There was no difference between the radiological and pathologic extent. On MRI, the coefficients of correlation between the extent of periosteal abnormality with the extent of bone marrow invasion and cortical destruction were 0.901 and 0.746 respectively. The coefficients of correlation between the extent of periosteal abnormality with the longitudinal diameter and transverse diameter of the soft tissue mass were 0.717 and 0.100 respectively. Conclusion MRI can demonstrate the extent of periosteal abnormality accurately, which correlates well with the extent of bone marrow and cortical invasion as well as the size of the soft tissue mass.
