预防性横结肠造口在腹腔镜辅助直肠癌低位前切除术中的应用研究

目的探讨预防性横结肠造口在预防腹腔镜辅助直肠癌低位前切除术后吻合口漏发生的临床价值。方法分析107例腹腔镜辅助直肠癌低位前切除术患者术后吻合口漏发生情况,其中行预防性横结肠造口患者47例,未行预防性横结肠造口患者60例。结果行预防性横结肠造口术组患者吻合口漏发生率为2.1%,平均住院时间为（16.1±0.2）d;未行预防性横结肠造口组吻合口漏发生率为13.3%,平均住院时间为（24.2±0.1）d,两组吻合口漏发生率差异有统计学意义（P〈0.05）。结论在腹腔镜辅助直肠癌低位前切除术中行防性横结肠造口能降低吻合口漏的发生率。

Study on the Application of Defunctioning Transversostomy in Laparoscopic Low Anterior Resection of Rectal Cancer

Objective To study the clinical value of defunctioning transversostomy in the prevention of anastomotic leakage in laparoscopic low anterior resection of rectal cancer. Methods Anastomotic leakage was observed in a total of 107 rectal cancer patients who underwent laparoscopic low anterior resection.Among the patients,47 were subjected to the defunctioning transversostomy and 60 not. Results The incidence of anastomotic leakage of the group undergoing defunctioning transversostomy was 2.1%,the average length of stay was 16.1±0.2d（14-18d）; while that of the group without defunctioning transversostomy was 13.3%, the average length of stay was 24.2±0.1d（22-30d）, the difference in the incidence of anastomotic leakage between the two groups was statistically significant（P〈0.05）. Conclusion Defunctioning transversostomy in laparoscopic low anterior resection of rectal cancer can reduce the incidence of anastomotic leakage.
