高血压患者认知功能障碍与Hcy、HIF-1α、CCL2水平的关系研究

摘    要:目的观察高血压患者血清同型半胱氨酸(Hcy)、缺氧诱导因子1α(HIF-1α)、CC趋化因子配体2(CCL2)的水平变化及其与认知功能的关系。方法选取2017年1月至2019年1月,重庆市大足区人民医院就诊的高血压患者202例,根据认知功能水平将所入选高血压患者分为2组:认知功能正常(A组,100例)与认知功能障碍非痴呆(B组,102例)。选择同期于医院体检的正常血压人群为对照组(100例)。收集各组基线临床资料,检测各组的血清Hcy、HIF-1α、CCL2,采用简易精神状态量表(MMSE)检查各组的认知功能,比较各组间上述观察指标的差异,分析高血压患者血清Hcy、HIF-1α、CCL2水平变化及其与认知功能的关系。结果与对照组相比,A组、B组的高血压患者血清Hcy、HIF-1α、CCL2水平升高,且B组较A组升高显著(均P<0.05);与对照组相比,A组、B组的高血压患者MMSE评分降低,且B组较A组降低显著(均P<0.05)。Pearson相关性分析结果显示,Hcy、HIF-1α、CCL2水平和MMSE评分呈负相关(r=-0.46,P<0.01;r=-0.34,P<0.01;r=-0.44,P<0.01)。高血压患者认知功能与血清Hcy、HIF-1α、CCL2水平相关(均P<0.05)。结论高血压患者认知功能障碍可能与血清Hcy、HIF-1α、CCL2水平的升高相关。

Abstract:Objective To observe changes of serum homocysteine(Hcy),hypoxia inducible factor-1 alpha(HIF-1 alpha)and CC chemokine Ligand-2(CCL2)levels in patients with hypertension and their relationships with cognitive function.Methods 202 patients with hypertension were selected from the First Hospital of Quanzhou City from January,2017 to January,2019.According to the level of cognitive function,the patients were divided into two groups:the normal cognitive function group(group A,100 cases)and the non-dementia with cognitive impairment group(group B,102 cases).The control group(100 cases)was selected from patients with normal blood pressure during the same period from hospital physical examination.The baseline clinical data of each group were collected,and the serum Hcy,HIF-1αand CCL2 levels were detected.The cognitive function of each group was examined by MMSE.The differences of the above-mentioned indexes were compared among the groups,and the changes of serum Hcy,HIF-1αand CCL2 levels in patients with hypertension and their relationships with cognitive function were analyzed.Results Compared with the control group,the levels of serum Hcy,HIF-1αand CCL2 in group A and B were increased significantly(all P<0.05).Compared with the control group,the MMSE scores of hypertension patients in group A and B were decreased,and the MMSE scores of hypertension patients in group B were significantly lower than those in group A(all P<0.05).Pearson correlation analysis showed that Hcy,HIF-1α,CCL2 levels were negatively correlated with MMSE scores(r=-0.46,P<0.01;r=-0.34,P<0.01;r=-0.44,P<0.01).Cognitive function was correlated with serum levels of Hcy,HIF-1αand CCL2 in patients with hypertension(all P<0.05).Conclusion Cognitive dysfunction in patients with hypertension may be related to the elevation of serum Hcy,HIF-1αand CCL2 levels.
