腹腔镜手术与开腹手术治疗急性阑尾周围脓肿疗效比较

目的：比较腹腔镜手术和开腹手术治疗急性阑尾炎周围脓肿的临床疗效。方法将120例急性阑尾炎周围脓肿患者随机分为观察组、对照组，每组60例。两组患者均给予全麻，采用头低足高位。观察组采用腹腔镜手术治疗，对照组采用传统开腹治疗。两组均于术后给予抗生素对症治疗5d。观察两组手术持续时间、引流时间、切口长度、术后卧床时间、术后镇痛剂使用情况、肛门排气时间以及残端出血、切口感染、肠梗阻等并发症发生情况。结果两组患者各60例均顺利完成手术，手术成功率达100.00%。下床时间、住院时间、术后镇痛药使用率、治疗总费用、肛门排气时间、体温恢复正常时间、白细胞恢复正常时间、并发症发生情况等术后相关指标两组相比有显著性差异（P＜0.05）。结论与开腹手术相比腹腔镜手术治疗急性阑尾炎，具有创伤小、术后恢复快、术后并发症少等优势。

Comparison of laparoscopic operation and laparotomy in the treatment of acute periappendicular abscess efficacy

Objective To compare the clinical effect of laparoscopic operation and open operation in treatment of acute appendicitis abscess. Methods 120 cases of acute appendicitis abscess were randomly divided into observation group and control group, each group of 60 cases. 2 groups of patients were given anesthesia, using the Trendelenburg position. The observation group was treated with laparoscopic operation treatment, the control group using the traditional laparotomy. 2 groups in the postoperative treated with antibiotics for 5 days. Observed 2 groups of operation duration, drainage time, incision length, postoperative bed time, postoperative analgesic use, anal exhaust time and stump bleeding, wound infection, intestinal obstruction and other complications. Results The 2 groups of the 60 cases of patients were successfully completed operation, operation success rate of 100%. Bed time, hospitalization time, postoperative analgesic use related index, the total cost of treatment, anal exhaust time, body temperature returned to normal time, white blood cell recovery time, complications after operation in the 2 groups had significant difference （P〈0.05）. Conclusion Compared with open operation of laparoscopic operation in the treatment of acute appendicitis, with less trauma, faster postoperative recovery, less postoperative complications and other advantages.
