同期适形放疗化疗治疗中晚期中段食管癌临床研究

目的 观察同期化疗和适形放疗治疗晚期中段食管癌的近期疗效和毒副反应。方法 20例中晚期中段食管癌予以同期适形放疗和DF方案化疗。结果 全部病人按计划完成治疗。急性放射性食管炎发生率1～2级80％(16／20)、3级35％(7／20)；放射性气管炎发生率1～2级20％(4／20)．3级5％(1／20)；急性骨髓抑制发生率为95％(19／20)、3～4级65％(13／20)；总有效率为100％，其中完全缓解(CR)80％(16／20)，部分缓解(PR)20％(4／20)。1、2年的生存率分别为65％(13／20)、35％(7／20)。结论 三维适形放疗同时配合化疗对中晚期中段食管癌有较好的近期疗程．早期并发症较低，能为患者耐受，晚期并发症的发生率及远期疗效有待进一步观察。

Concomitant three Dimensional Conformal Radiotherapy Plus Chemotherapy for Advanced Esophageal Carcinoma

Objective To evaluate the response and acute toxicity of three dimensional conformal radiotherapy (3DCRT) plus chemotherapy for advanced esophageal carcinoma. Methods 20 patients with advanced esophageal carcinoma were treated with three dimensional conformal radiotherapy combined with chemotherapy DF(DDP and 5-FU), Results All patients completed treating schedule.Grade 1-2 acute radiation esoph-agitis occurred in 80%(16/20) and grade 3 in 35%(7/20) patients: Grade 1-2 acute radiation bronchitis occurred in 20%(4/20) and grade 3 in 5%(1/20) patients; Acute bone marrow suppression occurred in 95%(19/20) and grade 3-4 in 65% (13/20)patients. The overall response rate was 100%, with complete response (CR) 80% (16/20) and partial response (PR) 20% (4/20),the 1-and 2-year survival rates were 65% (13/20), 35%(7/20).Conclusion Three dimensional conformal radiotherapy (3DCRT) plus chemotherapy can be considered effective advanced esophageal carcinoma. With fewer acute complications. Complications and long-term survival require further study.
