大肠癌病人血清胃泌素和癌胚抗原测定及其临床意义

目的探讨血清胃泌素（ＧＡＳ）、癌胚抗原（ＣＥＡ）对大肠癌的诊断价值。方法采用放射免疫法（ＲＩＡ）测定大肠癌（６０例）、胃癌（４０例）、肝癌（３０例）、消化系统良性疾病（４０例）及正常人（３０例）的ＧＡＳ与ＣＥＡ水平。结果大肠癌组、胃癌组ＧＡＳ均显著高于正常组（Ｐ＜０．０１），两癌组间差异不显著（Ｐ＞０．０５），肝癌组、消化系良性疾病组与正常组间差异无显著性（Ｐ＞０．０５）。大肠癌组ＧＡＳ术后显著下降（Ｐ＜０．００１）。不同病变部位ＧＡＳ、ＣＥＡ差异无显著性（Ｐ＞０．０５）。分化良好腺癌的ＧＡＳ、ＣＥＡ均稍高于中、低分化癌。结论血清ＧＡＳ、ＣＥＡ检测可做为普查筛选大肠癌的一项客观指标，但ＣＥＡ敏感性很低，远逊于ＧＡＳ，胃癌组与大肠癌组间有交叉重叠，须注意鉴别。ＧＡＳ对判断手术疗效及细胞分化程度有参考价值。

Detection and Clinical Significance of Serum Gastrin and Carcinoembryonic Antigen in Patients with Colorectal Cancers

To explore the diagnostic value of serum gastrin(GAS)and carcinoembryonic antigen(CEA) in colorectal cancer.Method Radioimmunity(RIA)was used for the detection of GAS and CEA of 60 patients with colorectal cancer,40 patients with gastric cancer,30 patients with liver cancer,40 patients with benign digestive disease and 40 normal controls.Results GAS in both colorectal cancer group and gastric cancer group was significantly higher than that of normal control(P<0.01). There was no difference between these two cancers(P>0.05),also among liver cancer,benign gastrointestinal diseases and normal control (P>0.05).GAS of postoperative patients with large colorectal cancer was significantly lower than that of preoperative patients.GAS and CEA in well-differential adenocarcinoma were slighty higher than those in poor,differential one.But there was no difference among different regions.Conclusion Detection of serum GAS and CEA will be Abstract items in the screening colorectal cancer.The sensitivity of CEA is obviously lower than that of GAS.There is overlap between gastric cancer and colorectal cancer,so differential diagnosis by other means is required. GAS is also valuable for determination of the operative effect and degree of cancer differentiation.
