槟榔提取物对口腔黏膜成纤维细胞表达细胞间粘黏分子-1的影响(英文)

摘    要:目的咀嚼槟榔与口腔黏膜下纤维性变(OSF)有关,但其相关机制尚不清楚。观察槟榔提取物(ANE)对体外培养的人类口腔黏膜成纤维细胞(FB)表达细胞间黏附分子-1(ICAM-1)的影响,探讨FB与ICAM-1在OSF发生中的作用。以槟榔提取物对体外人口腔黏膜成纤维细胞表达细胞间黏附分子的影响,证明咀嚼槟榔与OSF发病机制相关的若干假设成立。方法所有口腔黏膜标本都采自1997-07/1998-04在中南大学湘雅二医院口腔科就诊的患者,经临床及病理确诊为OSF者(男8例,女3例)作为实验组。另外,从同时间来院的手术患者中,选择无槟榔、烟酒及辣椒嗜好,年龄与实验组匹配者(男6例,女5例),作为对照组。分别从正常(NM)及OSF患者的口腔黏膜中培养出FB,向培养基中加入不同浓度的ANE培养48h后,用细胞酶联免疫方法检测FB表达的ICAM-1水平。结果表示ICAM-1水平高低的OD值在OSF-FB为0.386±0.099,高于NM-FB的OD值0.324±0.030(F=18.177,P=0.001);ANE在50~150μg/ml内以浓度-效应依赖关系刺激FB产生ICAM-1。结论ANE能上调FB表达ICAM-1的水平,从而间接促进纤维化过程,引起OSF的发生。

Abstract:Aim Areca nut chewing has been associated with the incidence of oral submucous fibrosis (OSF),however the related mechanism is not clear.The study aimed to investigate the roles of fibroblasts and intercellular adhesion molecular-1(ICAM-1) in the pathogenesis of OSF and to evidence the hypotheses related the mechanism of OSF induced by areca nut chewing by comparing the difference of ICAM-1 expression in fibroblasts obtained from oral submucous fibrosis buccal mucosa and normal buccal mucosa and observing the effects of areca nut extracts (ANE) on the expression of ICAM-1 in oral fibroblasts.Methods All subjects were treated at the Department of Stomatology,the Second Xiangya Hospital,Central South University,Changsha,China during the period from 1997 to 1998.Eleven specimens were obtained from OSF patients (male 8,female 3).All subjects had written proper informed consent.Diagnosis of OSF was based on clinical and histological features.Eleven biopsy specimens of normal buccal mucosa (male 6, female 5) were obtained from non-Areca nut chewers who had not the abuse of alcohol and chili and were used as controls.Fibroblasts were obtained from normal buccal mucosa (NM-FB) and OSF buccal mucosa (OSF-FB) and cultured in vitro.Then the expression of ICAM-1 incubated with or without areca nut extract in the presence of 10% fetal calf serum for 48 h at 37 ℃ in 5% CO2 and air were measured by using enzyme-linked immunosorbent assay (ELISA).Results OSF-FB expressed ICAM-1 at high level (0.386± 0.099) as compared to that of NM-FB (0.324± 0.030) (F=18.177, P=0.001).Areca nut extracts (50- 150 μ g/ml) up-regulated ICAM-1 expression in a concentration-dependent manner in both cell types.Conclusion These data suggest that areca nut components might induce the development of OSF by up-regulating ICAM-1 expression of FB and indirectly stimulating the fibrotic process.
