小剂量地塞米松对开胸患者手术期血糖的影响

摘    要:目的: 探讨小剂量地塞米松对开胸患者手术期血糖的影响。方法: 择期全麻下行开胸手术的患者90例,随机分为3组(每组30例):生理盐水组,地塞米松5 mg组及地塞米松10 mg组。于麻醉诱导前,生理盐水组静脉给予生理盐水2 mL,地塞米松5 mg组及10 mg组分别静脉给予地塞米松5、10 mg(2 mL)。分别于麻醉诱导前及麻醉诱导后1, 3, 8 h测定血糖。记录各组的血糖值及高血糖的发生率(在任一时间点血糖高于10 mmol/L)。结果: 3组在3 h及8 h的血糖明显高于麻醉诱导前的基础值;但在任一时间点上3组间的血糖水平及各组高血糖的发生率差异无统计学意义。结论: 全麻诱导前给予小剂量地塞米松不影响开胸患者手术期的血糖水平。

Abstract:Objective: To determine the effect of low-dose dexamethasone on blood glucose concentrations in thoracic surgery patients during the first 8 hours after administration. MethodsNinety patients undergoing selective thoracic surgery were divided randomly into three groups: saline group, dexamethasone 5 mg group, and dexamethasone 10 mg group. Before anesthesia induction, dexamethasone 5 mg and 10 mg were injected intravenously in relative group, respectively. Normal saline was given in a group as control. Blood glucose concentrations were measured at baseline (before anesthesia induction) and 1, 3, and 8 hours after administration. The incidence of hyperglycemic events (At any giving time blood glucose concentration>10 mmol/L) was determined. ResultsCompared with baseline, blood glucose concentrations in three groups significantly increased at 3 h and 8 h timepoints. But, the blood glucose concentrations did not differ significantly between the groups receiving dexamethasone (either 5 or 10 mg) and those receiving saline at any measurement time. The incidence of hyperglycemic events did not differ in three groups. Conclusions: Administration of low-dose dexamethasone before anesthesia induction did not influence blood glucose level in thoracic surgery patients during the first 8 hours after administration.
