早期肠内肠外联合营养对机械通气慢性阻塞性肺疾病患者的影响

目的:观察早期肠内肠外联合营养对慢性阻塞性肺病(COPD)患者的机械通气时间、胃肠功能、营养状况的影响。方法:28例入住ICU需机械通气的慢性阻塞性肺病患者,随机分为两组,分别给予早期肠内与肠外联合营养支持(EN+PN组,13例)及肠内营养支持(EN组,15例)。观察营养支持观察两组间胃肠功能情况及患者机械通气时间及预后情况。结果:1周内,EN+PN组患者13例未发生胃肠道不耐受症状;EN组15例发生腹胀及胃潴留5例,腹泻4例。1周后,EN+PN组患者血清白蛋白水平较营养支持前有明显升高,差异有统计学意义(P<0.05)。EN+PN组患者成功撤机10例(76.92%),机械通气时间为(9.4±2.6)d,EN组患者成功撤机11例(73.33%),机械通气时间为(11.6±1.8)d。两组成功撤机例数比较无差异,EN+PN组机械通气时间明显短于EN组,差异有统计学意义(P<0.05)。结论:肠内肠外联合营养支持较完全肠内营养支持的胃肠耐受性良好,能较好的保证机械通气的慢性阻塞性肺病患者营养需求,缩短机械通气时间。
