| 摘 要： | 目的探讨环形电切术(loop electrosurgical excision procedure,LEEP)治疗宫颈上皮内瘤样病变(cervical intraepithelial neoplasia,CIN)Ⅱ级的效果.方法28例阴道镜下宫颈活检病理学诊断为CINⅡ级,其中高危型人乳头瘤病毒(human papillomavirus,HPV)DNA阳性率96.4%(27/28).表面麻醉后环形电极切割宫颈组织,再用方形或小环形电极切除中央部分组织,包括部分颈管,术后随访.结果术中出血0～20 ml,手术时间5～10 min,术后3例阴道出血相当于月经量,无其他合并症.手术标本无明显炭化,15例(53.6%)CIN病灶消失,病变级别降低为CIN Ⅰ级5例,级别不变5例,级别增加至CINⅢ级3例.28例随访6～24个月,平均16个月.28例治愈,未发现CIN病变残留和复发,高危型HPV DNA转阴23例.结论LEEP是治疗CINⅡ级安全有效的方法,其优点是微创,手术操作简便,诊断与治疗能同时进行,无须住院.
