| 摘 要： | 目的:介绍通过自制简便、实用的经脐单孔器械,成功进行胃肠肿瘤手术的临床应用经验及其围手术期的营养管理。方法:5例病人分别为远端胃次全、胃局部切除、空肠部分切除、右半结肠切除、直肠腹前切除各1例,均行经脐单孔腹腔镜手术,遵循快速康复外科的原则,进行围手术期营养管理。结果:5例病人脐下手术切口3 cm,愈合良好。平均手术时间为138(60~240)min,平均出血量为50(10~100)ml。术后第1天均可下床活动,少量饮水,平均术后3 d恢复半流质饮食,平均术后住院时间为4 d,未发生术后并发症。出院时,体重与术前相比,平均减少约2.6 kg。结论:采用经脐单孔腹腔镜技术进行胃肠手术,具有切口小、美容效果好、创伤小和恢复快的优点。在快速康复外科概念指导下进行围手术期营养管理,术后体重丢失少。
