脊髓髓内肿瘤31例显微外科治疗分析

目的评估显微外科治疗脊髓髓内肿瘤疗效。方法回顾分析本院1996年7月至2002年10月采用显微外科技术切除31例脊髓髓内肿瘤的经验。结果室管膜瘤占51.6%,星形细胞瘤占32.3%,脂肪瘤占6.5%,上皮样囊肿占6.5%。全组手术全切除率为64.5%,16例室管膜瘤全切除,10例星形细胞瘤的全切除率为40%,近全切除率为30%,部分切除率为30%,2例脂肪瘤行大部分切除,1例转移瘤和2例上皮样囊肿为近全切除。结论脊髓髓内肿瘤以低恶性度肿瘤多见,手术效果较好,应尽早手术。

Microsurgery for treatment of 31 cases of intramedullary tumors in spinal cord

Objective To assess the curative effect of microsurgery for the treatment of intramedullary tumors in spinal cord.Methods Experiences of 31 cases of intramedullary tumors in spinal cord resected by using microsurgery were analyzed retrospectively from July 1996 to October 2002.Results Ependymoma accounted for 51.6%,astrocytoma accounted for 32.3%,lipoma accounted for 6.5% and epidermoid cysts accounted for 6.5%.The rate of total resection in the group was 64.5%.Sixteen cases of ependymoma was total resection.The rate of total,subtotal and partial resection of 10 cases of astrocytoma was 40%,30% and 30%,respectively.Two cases of lipoma were performed with subtotal resection.One case of metastatic tumor and two cases of epidermoid cysts were subtotal resection.Conclusion Low-grade malignant tumors were predominant in intramedullary spinal cord tumors.It is effective of operation for the treatment of intramedullary spinal cord cysts.It is recommended that operation should be performed as soon as early.
