胸壁挤压吸痰法用于感染性多发性神经根炎患儿效果观察

目的探讨胸壁挤压法在感染性多发性神经根炎气管切开并机械通气患儿排痰中的应用及效果。方法将49例感染性多发性神经根炎需气管切开并机械通气的患儿，按住院先后单号分为干预组（25例），双号分为对照组（24例），两组接受相同的常规治疗，均雾化吸入氟霉松和肝素钠，均于体位引流后吸痰，吸痰前后大流量吸氧3～5min。干预组在此基础上吸痰前予以胸壁挤压。结果干预组患儿排痰后的血氧饱和度上升幅度、肺部听诊结果，并发症及刺激性咳嗽发生率与对照组比较，差异有统计学意义（P〈0．05，P〈0．01）。结论胸壁挤压吸痰法用于感染性多发性神经根炎机械通气患儿，可提高吸痰效果。

Application of Sputum Suctioning Through Extruding the Thoracic Wall for Management of Infectious Polyradiculitis Children

Objective To explore the feasibility of sputum suctioning through extruding the thoracic wall for management of infectious polyradiculoneuritis children who received tracheotomy and mechanical ventilation. Methods Totally, 49 infectious polyradiculoneuritis children who received tracheotomy and mechanical ventilation were divided into an interventional group （25 cases） and a control group （24 cases） based on the odd and even dates of admission. Both groups received conventional treatment, including aerosol inhalation of dexamethasone acetate and heparin sodium, postural drainage and then sputum suctioning. Before suctioning, all pa- tients inhaled high flow rate oxygen for 3-5 rain. Besides that, patients in the interventional group received sputum suctioning through extruding the thoracic wall. Results The increase in oxygen saturation, results of pulmonary auscultation, rates of complications and irritating coughing in both groups were significantly different（P〈0.05, P〈0.01）. Conclusion The application of sputum suctioning through extruding the thoracic wall is effective and helos optimize suction result.
