动态增强成像在鉴别乳腺良恶性肿瘤中的价值

摘    要:目的 探讨动态增强成像在鉴别乳腺良恶性肿瘤中的价值.方法 39例乳腺疾病患者行MR平扫和动态增强成像及延迟扫描,进一步分析每个病灶的时间信号强度曲线类型、定量分析动态增强成像达到强化峰值的时间,进行统计学处理.结果 共检出46个病灶,其中良性18个、恶性28个,良、恶性病灶的时间信号强度曲线具有特异性,良性病灶多为I、II型,其中I型有15个、II型2个,恶性病变多为II、III型,其中II型有3个、III型有24个,良、恶性病灶在强化峰值时间上的差异具有统计学意义(P<0.05),以180s为强化峰值时间阈值来鉴别良、恶性病灶,灵敏度最高,达90.87%.结论 利用动态增强成像的时间信号强度曲线和强化峰值时间来鉴别乳腺良、恶性病灶,敏感性和特异性较高,具有一定的临床应用价值.

Abstract:Objective: To study the value of dynamic contrast enhanced imaging in differentiating the benign and malignant tumors of breast.Methods: 39 patients with breast disease were underwent plain MR,dynamic contrast enhanced MR imaging and delay scanning,further analysis to the time signal intensity curve for each type of lesion,quantitative analysis to the peak time to enhanced of dynamic contrast enhanced imaging,and have statistical analysis to them.Results: 46 lesions were detected,include 18 benign and 28 malignant,the time signal intensity curve of benign and malignant lesions have specific,the type of benign lesions,were type I,II mostly,15 lesions were type I,2 lesions were type II,the type of malignant lesions were type II,III mostly,3 lesions were type II,24 lesions were type III,the peak time to enhanced of benign and malignant lesions have statistically difference(P0.05),look 180 s as the peak time to enhanced threshold,have the highest rate to differential diagnosis of benign and malignant lesions,was 90.87%.Conclusion: Using the time signal intenstiy curve and the peak time to enhanced of dynamic contrast enhanced imaging in differentiating benign and malignant lesions of breast,have high sensitivity and specificity,and have some clinical value.
