门诊结核病患者实施全程管理效果分析

目的:探讨结核病患者全程管理对相关指标变化的影响。方法:对我院从2008年下半年开始实施全程管理的患者进行相关指标的调查,观察患者的2个月痰菌转阴率、治愈率、失败率和丢失率等,选择初治涂阳病例与实施全程管理之前3年的病例进行比较,分析其对结核病相关指标的影响。结果:初治涂阳结核病患者实施全程管理后的3年较之前的3年各项指标均有好转,2个月痰菌转阴率从29.0%上升到46.6%,治愈率从20.6%上升到61.5%,完成疗程率从50.3%下降到15.0%,失败率从11.9%下降到4.2%,丢失率从14.1%下降到3.8%,正常迁出率从2.1%上升到12.0%。各项指标比较差异均有统计学意义(P均<0.01)。结论:实施全程管理在肺结核的治疗过程中,可增加患者遵医依从性和提高临床综合评价指标。

Effect of full management of TB patients in clinic

Objective To explore the effects of full management of TB patients on related indicators changes.Method The related indicators of TB patients received full management hospitalized from the second half of 2008 were investigated,the sputum negative conversion rate in 2 months,the cure rate,failure rate and loss rate were observed,new smear positive cases were compared with those cases treated 3 years before the full management,and its impact on TB-related targets was analyzed.Results The indicators of new smear positive cases received 3 years of full management were improved compared with 3 years ago,the sputum negative conversion rate in 2 months was from 29.0% to 46.6%,the cure rate from 20.6% to 61.5%,completed treatment rate from 50.3% to 15.0%,failure rate from 11.9% to 4.2%,loss rate from 14.1% to 3.8%,normal moving out rate from 2.1% to 12.0%.Comparison of the indicators showed significant differences(P<0.01).Conclusion Full management in the treatment of pulmonary tuberculosis can increase compliance of patients and improve clinical indicators evaluation.
