预防性护理干预降低PICC导管致机械性静脉炎的效果评价

目的探讨PICC致机械性静脉炎的预防措施。方法将80例行PICC的肿瘤患者按置管序号单双数分为观察组和对照组,各40例。观察组术前采用地塞米松肝素钠混合液浸泡导管,术后24h于穿刺部位上方近心端(沿血管走向)2～10cm处给予75%酒精湿敷。对照组采用常规方法置管。观察1周内两组穿刺部位机械性静脉炎发生情况。结果观察组机械性静脉炎发生率为5.0%,对照组发生率为22.5%,两组比较有显著性差异(P<0.05)。结论地塞米松肝素钠混合液浸泡导管联合术后酒精湿敷,可有效预防PICC所致机械性静脉炎的发生,具有操作简单、价格低廉、效果好等优点。

Effects Evaluation on Protectively Nursing Intervene Treatment Decreasing the Rate of Mechanical Phlebitis after PICC

Objective To discover the clinic effect through the pre--treatment of mechanical phlebitis induced by PIG（2. Methods 80 patients with mechanical phlebitis after PICC were divided into the Observation Group （40 cases） and the Comparison Group （40 cases） as per the Single or Even No. The 40 eases in Observation Group were treated by putting the catheter into the mixture of Dexamethasone and Heparin Sodium before PICC. And once the catheter removed after 24hs, 75% spirit moisture compress adopted above 2--10 CM closing to the heart at the puncturing pot along the vein. The patients in comparison Group applied the normal PICC treatment without any other special method. Monitored and recorded strictly the occurrence rate of mechanic phlebitis within one week. Results The rate was 5. 0% in Observation Group,and 22. 5% in Comparison Group. The difference rate is of statistical significance （P〈0. 05）. Conclusion the therapy combined with catheter marinated into mixture of Dexamethasone and Heparin Sodium before PICC and after the treatment through moisture compress can effectively protect mechanical phlebitis in- duced hv PICC. It is easily operated and economicallv cheaper and with verv effective result.
