| 摘 要： | 介绍杨浦区在实施以全科团队为载体的社区卫生服务模式的管理机制及成效。该管理机制主要包括创新性建立全科团队日常工作记录制度，配套相应的培训、考核和激励等措施，促进全科团队步入健康发展的轨道。可为社区卫生服务全科团队管理机制的日趋完善提供参考。
