维生素D联合阿法骨化醇治疗骨质疏松症的临床研究

目的探讨维生素D滴剂联合阿法骨化醇软胶囊治疗骨质疏松症的临床疗效。方法选取2018年1月—2019年12月因骨质疏松症来河南省荣军医院接受治疗的134例患者为研究对象,将所有患者随机分为对照组和治疗组,每组各67例。对照组口服阿法骨化醇软胶囊,2粒/次,1次/d。治疗组在对照组治疗的基础上口服维生素D滴剂,400 IU/次,1次/d。两组患者的治疗时间均为3个月。观察两组的临床疗效,比较两组治疗前后生活质量、骨代谢指标、疼痛情况、行走恢复情况。结果治疗后,对照组和治疗组的总有效率分别为77.6%、94.0%,与对照组比较治疗组总有效率明显较高(P<0.05)。治疗后,两组的社会功能、躯体功能、角色功能、认知功能评分均显著升高(P<0.05),与对照组比较治疗组社会功能、躯体功能、角色功能、认知功能评分均明显较高(P<0.05)。治疗后,两组骨钙素(BGP)、甲状旁腺激素(PTH)、碱性磷酸酶(AKP)水平均降低,25羟基维生素D(25-OH-D)水平升高,治疗前后比较差异有显著意义(P<0.05);治疗后,与对照组比较治疗组BGP、PTH、AKP水平均降低,25-OH-D水平升高(P<0.05)。与治疗前比较,治疗后两组的视觉模拟疼痛量表(VAS)评分均降低(P<0.05);治疗后治疗组VAS评分与对照组比较显著较低(P<0.05)。治疗后,与对照组恢复行走率(80.6%)相比,治疗组恢复行走率(97.0%)明显较高(P<0.05)。结论维生素D滴剂联合阿法骨化醇软胶囊治疗骨质疏松症临床效果较好,可明显改善骨代谢、疼痛情况,有效恢复患者行走功能,明显提高生活质量水平。

Clinical study on vitamin D combined with alfacalcidol in treatment of osteoporosis

Objective To investigate the clinical effect of Vitamin D Drops combined with Alfacalcidol Soft Capsules in treatment of osteoporosis. Methods Patients (134 cases) with osteoporosis in Henan Rongjun Hospital from January 2018 to December 2019 were randomly divided into control and treatment groups, and each group had 67 cases. Patients in the control group were po administered with Alfacalcidol Soft Capsules, 2 grains/time, once daily. Patients in the treatment group were po administered with Vitamin D Drops on the basis of the control group, 400 IU/time, once daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and quality of life, bone metabolism index, pain, and walking recovery in two groups were compared. Results After treatment, the total effective rates of the control group and the treatment group were 77.6% and 94.0%, respectively, and there was significantly difference between two groups (P<0.05). After treatment, the scores of social function, physical function, role function, and cognitive function in two groups were significantly increased (P<0.05), and after treatment, the scores in the treatment group were significantly higher than those in the control group (P<0.05). After treatment, the levels of BGP, PTH, and AKP in two groups were decreased, while the levels of 25-OH-D in two groups were increased (P<0.05). After treatment, the levels of BGP, PTH, and AKP in the treatment group were significantly lower than those in the control group, while the level of 25-OH-D in the treatment group was significantly higher than that in the control group (P<0.05). After treatment, the VAS scores of two groups were decreased (P<0.05), and after treatment, the VAS score of the treatment group was significantly lower than that of the control group (P<0.05). After treatment, the recovery rate of walking in the treatment group (97.0%) was significantly higher than that of the control group (80.6%) (P<0.05). Conclusion Vitamin D Drops combined with Alfacalcidol Soft Capsules has clinical curative effect in treatment of osteoporosis, can significantly improve the bone metabolism and pain, effectively restore the walking function of patients, significantly improve the quality of life.
