MR T1rho成像诊断肝硬化及其与肝功能Child-Pugh分级的关系

摘    要:目的观察MR T1rho成像诊断肝硬化的可行性,分析其与Child-Pugh肝功能分级的关系。方法对42例肝硬化患者和22名健康成年志愿者行肝脏MR T1rho检查,对比T1rho值与Child-Pugh肝功能分级,拟合T1rho值诊断肝硬化的ROC曲线。结果Child-Pugh A、B、C级组肝T1rho值分别为(46.32±3.80)ms、(48.08±3.97)ms、(54.43±2.48)ms,均高于健康志愿者(41.45±2.51)ms(P均<0.01)。Child-Pugh A与C级、B与C级T1rho值差异均有统计学意义(P均<0.01),而A与B级T1rho值差异无统计学意义(P=0.17)。Child-Pugh分级与T1rho值呈正相关(r=0.81,P<0.01),根据肝T1rho值鉴别不同Child-Pugh分级肝硬化的AUC为0.82~0.95(P均<0.01)。结论MR T1rho成像可用于定量诊断肝硬化,并可初步评价肝硬化患者Child-Pugh肝功能分级。

Abstract:Objective To observe the feasibility of MR T1rho imaging in diagnosis of liver cirrhosis, and to analyze the relationship between T1rho value and Child-Pugh liver function classification. Methods Totally 42 patients with liver cirrhosis and 22 healthy volunteers underwent liver MR T1rho imaging. T1rho values were compared with Child-Pugh classification. ROC curves were calculated to evaluate the accuracy of T1rho values for assessment of different Child-Pugh classification. Results T1rho values of patients with Child-Pugh class A, B, C was (46.3±3.80) ms, (48.08±3.97) ms and (54.43±2.48) ms, respectively, all higher than that of volunteers (ms). There were significant differences of T1rho values between Child-Pugh class A and C (P<0.01) as well as between B and C (P<0.01), but not between class A and C (P=0.17). There was positive correlation between Child-Pugh classification and T1rho values (r=0.81, P<0.01). AUC of T1rho values in differentiating liver cirrhosis with different Child-Pugh grades were 0.84-0.95. Conclusion MR T1rho imaging was feasible to detect human liver cirrhosis, which might be used to Differentiate liver cirrhosis patients with different Child-Pugh grades.
