无乳头溢液的导管内乳头状瘤的超声图像特征分析

摘    要:目的 分析无乳头溢液的导管内乳头状瘤的超声图像特征。方法 回顾分析62例(73个病灶)经病理确诊的导管内乳头状瘤超声图像,将病灶分为5型:I型:扩张导管伴结节,II型:囊实混合性结节,III型:实性结节,IV型:囊性结节,V型:单纯导管扩张。结果 73个病灶中,I型27个,II型11个,III型30个,IV型3个,V型2个,超声对各型的诊断与病理符合率分别为:88.9%、90.9%、40.0%、0、0。结论 无乳头溢液的导管内乳头状瘤的典型声像图是扩张导管伴结节和囊实性混合结节,通过探索声像图特征有助于提高诊断准确率。

Abstract:Objective To investigate the imaging features of breast intraductal papilloma without nipple discharge on high-frequency and color doppler ultrasound. Methods This retrospective study evaluated 62 patients (73 lesions) without nipple discharge with pathologically-confirmed intraductal papilloma. According to the characteristics of the ultrasound images, 73 lesions were divided into 5 types. Type I:nodules accompanied with dilated ducts, Type II:cystosolid nodules, Type III:solid nodules, Type IV:cystic nodules, and TypeV:duct dilatation. Results Among the 73 lesions, 27 of them were classified into type I. And 11 lesions showed as cystosolid nodules were classified into type II.30lesionsconsisted of nodules without dilated ducts were classified into type III. 3 lesions showed as cystic nodules alone were classified into type IV. The other 2 lesions presented as dilated ducts only were classified into type V. The correct rates of ultrasound diagnosis of each type were: 88.9%, 90.9%, 40.0%, 0, 0, respectively. Conclusion The imaging features of intraductal papilloma without nipple discharge on ultrasound are diversity, and their typical features on ultrasound images are presented as nodules accompanied with dilated ducts and cystosolid nodules. Analysis of the imaging features on ultrasound may be beneficial to improve the accuracy of diagnosis of intraductal papilloma without nipple discharge.
