Abstract:Traumatic craniocervical arterial dissection (TCAD) refers to intermural hematoma formed by blood flowing into the vessel wall when intima of the craniocervical artery avulsed by trauma. While ischemic stroke is the most common clinical manifestation, TCAD is often overlooked due to lack of onset symptoms. Imaging examination is of great value in the diagnosis of TCAD, and computed tomography angiography (CTA) is the primary choice. Digital substraction angiography (DSA) is feasible when TCAD cannot be diagnosed by CTA. The current main treatments for TCAD include antithrombotic treatment and surgery. Emerging new treatment methods represented by intravascular treatment are less difficult and risky, and with lower complication incidence rate compared with traditional open surgery. In this article, we will review the progress in the diagnosis and treatment of TCAD.
