高三尖杉酯碱对慢性髓细胞白血病患者T细胞亚群和Th细胞亚群的影响

摘    要: 目的 研究慢性髓细胞性白血病(CML)患者高三尖杉酯碱(HHT)治疗前后细胞免疫功能的变化。方法 测定CML患者初诊时及HHT治疗后T细胞亚群和Th细胞亚群的变化并观察初诊患者外周血单个核细胞(PBMNC)在体外培养过程中Th细胞亚群的变化。结果 初诊时患者T细胞中以CD+4 T细胞占优势,Th亚群中以Th2细胞占优势,HHT治疗后达主要细胞遗传学反应患者则分别转变成以CD+8 T细胞和Th1细胞占优势。初诊患者PBMNC在培养前和培养早期以Th2细胞占优势,培养24 h和48 h则Th1细胞占优势。结论 CML患者存在免疫监视功能低下,HHT治疗获主要细胞遗传学反应患者的细胞免疫功能则明显恢复。

Abstract:Objective To investigate The changes of T and Th lymphocyte subsets in chronic myeloid leukemia (CML) before and after treatment with Homoharringtonine (HHT). Methods T and Th lymphocytes subsets of CML patients were measured at the began of diagnosis and one year later of treatment with HHT. PBMNC from primary diagnosis patients were cultured in RPMI1640 and the Th subsets were measured at 0 h, 4 h, 12 h, 24 h and 48 h respectively. Results At primary diagnosis, CD 4 T lymphocytes had an advantage in T lymphocyte subsets and Th2 cells had an advantage in Th lymphocyte subsets. The superiority subsets shifted from CD 4 T and Th2 lymphocyte into CD 8 T and Th1 subsets respectively in patients who got major cytogenetic response after treated with HHT. In cultured PBMNC, Th2 subsets were superiority at 0 h, 4 h and 12 h, but at the 24 h and 48 h after culture, The superiority subsets became Th1 subsets. Conclusion The cellular immunity descend remarkably at primary diagnosis and restores notably in patients who got major cytogenetics response after treatment with HHT.
