阴茎双功能超声检查在勃起功能障碍诊断中的应用

目的应用阴茎双功能超声检查结合阴茎海绵体血管活性药物注射(intracavernousinjection,ICI)评估勃起功能障碍(erectiledysfunction,ED)患者的阴茎动脉功能和静脉关闭机制。方法对320例ED患者采用百胜超声诊断仪(Biosound-AU4,USA)评估阴茎动脉功能和静脉关闭机制,ICI(复方罂碱注射液)诱导阴茎勃起,进行双功能超声阴茎血管功能血流动力学检查。结果阴茎双功能超声检查结果示非血管性ED216例(67·50%),动脉性ED62例(19·37%),静脉性ED42例(13·13%)。结论结合阴茎海绵体血管活性药物的应用,双功能超声检查在ED病因诊断中被广泛应用,是动脉性ED的“金标准”检查,是评估阴茎勃起各期动脉血流动力学变化最有价值的方法,并能筛选静脉性ED,为血管性ED首选的检查方法。

Role of Duplex Utrasonography in the Diagnosis of Erectile Dysfunction

Objective Use the duplex utrasonorgraphy and intracavernous injection to evaluate the hemodynamic change of cavernous arteries and venousocclusive mechanism.Methods Three hundred and twenty patients with erectile dysfunction (ED) were examined by duplex ultrasonography(Biosound-AU4,USA).Papaverine and Phentolamine were used for intracavernous injection to induce erection of penis.Results Two hundred and sixteen patients (67.50%) were nonvasculogenic ED.Sixty-two patients (19.37%) were arteriogenic ED and forty-two patients (13.13%) were venogenic ED.Conclusion Duplex ultrasonography plays an important role in the diagnostic workup of patients with erectile dysfunction.This accurate and reproducible method is useful to evaluate both arterial and venous hemodynamic abnormalities in patients with suspected vasculogenic ED.
