Dick氏钉复位内固定治疗胸腰椎骨折脱位并发瘫痪

作者应用Ｄｉｃｋ氏钉治疗胸腰椎骨折脱位并发瘫痪５１例。４６例获解剖复位，３例椎体楔变未完全矫正，１例前脱位未达解剖复位，１例定位错误，下位钉固定于骨折椎。５枚钉斜向椎间隙，余１９９枚钉均在无电视监视下准确经椎弓根进入椎体，无１例误入椎管或神经根管内。术后随访时间平均２．５年，无矫正度丢失及钉后移现象，但６例有镙帽松动。该内固定器对爆裂性骨折有一定的复位作用。Ｄｉｃｋ氏钉贯穿脊柱三柱有复位，固定双重功能，可重建脊柱稳定性，扩大椎管容积和促进脊髓，马尾神经功能最大程度地恢复。

Fiftv-one cases suffered from fract ure and/or dislocation of thoracolumbar vertebraewith paraplegia were treated surgically by Dick's internal fixer from 1989.The average duration ofFollow-up survey after operation was 2.5 years,Fortysix cases had achieved anatomical reduction,5cases acquired incompleted reduction,One hundred and ninetv-nine screws had beenscrewed into ver-tebrae accurately through vertebral pedicles,Not any one had been misled into the spinal canal or thecanal of nerve root.The fixation was no failure. Dick's internal fixer has the good function of reduc-tion and fixation and it could reconstruct spinal stability,due to passing through the spinaI three-col-umn,so the canal can be returned to normal,and the nervous function would recover to a great extent.
