病毒性脑炎的CT诊断

摘    要:目的:进一步认识病毒性脑炎的CT特征,以提高CT诊断的准确性,方法:回顾性分析19例病毒性脑炎的临床及CT资料,其中单纯疱疹病毒(HSV)脑炎13例,EB病毒(EBV)脑炎6例,均行CT平扫,16例行增强扫描,结果:13例HSV脑炎均累及颞叶和脑岛,单侧受累者11例,双侧2例,6例EBV脑炎均累及双侧基底节核团,其中4例累及双侧壳核,随访病变消失,另2例累及双侧豆状核和尾状核,随访仅见尾状核病变消失。结论:HSV脑炎及EBV脑炎的CT表现具有一定特征性,CT对判断EBV脑炎的预后有一定价值。

Abstract:Objective:To make a further recognition of CT characteristics of viral encephalitis to improve its diagnostic accuracy.Methods:The clinical materials and CT findings of 19 cases with viral encephalitis were analyzed retrospectively.Those patients included 13 of herpes simplex viral(HSV) encephalitis and 6 of Epstein Barr viral(EBV) encephalitis.Plain CT scans were performed in all patients and an additional enhanced CT scans in 16.Results:HSV encephalitis of 13 cases involved unilateral temporal lobe and insula in 11 cases and bilateral in 2.EBV encephalitis of 6 cases involved bilateral basal ganglia nucleuses,and additional bilateral putamen nucleuses were involved in 4 cases,which became normal on follow up.In another 2 cases,bilateral lenticular and caudate nucleuses were involved,but only caudate nucleuses involved became normal on follow up.Conclusion:HSV and EBV encephalitis have certain CT features. Moreover ,CT is valuable for evaluating the prognosis of EBV encephalitis.
