乳腺浸润性导管癌生长方式的体视学参数和淋巴结转移与预后关系的评价

摘    要:目的:评价乳腺浸润性导管癌生长方式的体视学参数及淋巴结转移情况与预后的关系。方法:根据体视学原理,采用模板法,测定癌实质体积密度(Vv)及癌巢的表面积密度(Sv)。观察H.E.切片进行淋巴结癌转移计数,比较Vv、Sv、淋巴结转移情况与预后的相关性。结果:在与预后有显著相关性的因素─淋巴结转移、癌实质体积密度(Vv)及癌巢的麦面积密度(Sv)中发现,Sv≤4.4的患者中,89.3%生存5年以上,Sv>5.5的患者中,82.5%死于5年以内。结论:癌肿生长方式的体视学参数─Sv值可作为独立判断预后的因素,Vv、淋巴结转移是乳腺浸润性导管癌预后的相关因素。

Abstract:Objective: Evaluate the value of the stereological parameter of growth pattem and lymph node status for the prognosis in breast invasive duct careinoma (IDC). Methods: Axillary lymph nodes status were assessed by observing H. E. sections for 343 cases of IDCs. Their volum density (Vv) of carinomas substance and surface area density (Sv) of carcinoma nests were measured according to the stereology principle and by means of pattem method,and the relationships between the variables and prognosis were compared respectively. Results: Lymph node status is differential with 5 - year stirvival rate significant statisically, and so are Vv, Sv. The Sv can signify IDCs prognosis indepdently by variables stratified analysis, 89. 3 pereent of the patients whose Sv figures are less than 4. 4 survived more than 5 years, while 82. 5 pement of the pahents whose Sv figures are more than 5. 5 survived less than 5 years. conclusion: Sv as a stercological parameter of growth pattem is reliable and independent prognostic factor.
