螺旋CT立体定位穿刺活检诊断肺部结节或肿块

目的探讨螺旋CT立体定位导引装置的准确性及对肺部结节或肿块病灶穿刺活检的临床诊断价值。方法 94例患者均在螺旋CT立体定位激光导引下行穿刺活检。其诊断结果与跟踪随访及手术病理诊断进行对比。结果全部患者均穿刺成功,其中63例一次进针即刺中目标(一针成功率 67．02％)。89例取得了具有诊断价值的组织、细胞学标本或涂片,活检成功率97．87％。91例行穿刺活检诊断中的75例与其获得随访的最终结果一致,诊断符合率为96．15％,诊断敏感性95．9％,特异性100％。结论螺旋CT立体定位是一种超选择性导引装置,能有效地提高穿刺活检的准确性和安全性,对肺部局限性肿块的定性诊断具有显著的临床应用价值。

Stereotactic puncture biopsy guided by spiral CT in diagnosis of localized neoplasm mass of lung

Objective To explore the clinical application value and accuracy of the stereotactic puncture biopsy guided by spiral CT in diagnosing lung nodes or neoplasm mass.Methods The 94 patients with hardly diagnosed lung node or neoplasm mass were subjected to Pinpoint stereotactic laser-guided by spiral CT percutaneous puncture biopsy.The diagnostic outcome of puncture biopsy was compared with results of clinical follow-up survey and operation, pathology.Results All the patients acquired puncture biopsy with success, among whom 63 were punctured accurately in focus one -time. The one -time successful rate was 67.02%;the valuable histiocyte sample or smear were acquired in 89 patients,biopsy successful rate was 97.87%;diagnosis outcome of 91 patients was identical the puncture biopsy in accord with the final outcome of follow-up.The diagnosis echo rate was 96.15%,sensitivity was 95.9%.specificity was 100% .Conclusion The stereotactic technique of spiral CT is a ultraselective guiding one.Its accuracy and safety coefficient can be effectually increased with puncture biopsy.It has significant applying value for qualitative diagnosing lung localized neoplasm mass.
