不同治疗方法对幽门螺杆菌清除率的影响

目的 观察既有牙周炎又有消化性溃疡的患者,使用不同的治疗方法,对幽门螺旋杆菌清除率的影响.方法 选则既有牙周炎又有消化性溃疡的43名患者,随机分为2组,实验组23人接受三联药物和牙周基础治疗；对照组20人仅接受三联药物治疗.治疗前和治疗结束3、6个月,分别用PCR方法检测患者牙周袋和胃部幽门螺杆菌情况,计算其清除率,并比较组间差异.实验组同时观察牙周临床指数的变化.结果 实验组胃部幽门螺杆菌治疗后6个月的清除率高于对照组（P＜0.05）.实验组口腔幽门螺杆菌治疗后3、6个月的清除率均高于对照组（P＜0.05）.实验组牙周治疗后3、6个月探诊深度、附着丧失水平与治疗前比明显好转（P＜0.05）.结论 对既有牙周炎又有消化性溃疡的患者,进行三联药物治疗的同时应进行牙周基础治疗,可提高幽门螺杆菌的清除率.

Effect of different treatment on eradication of Helicobacter pylori

Objective To evaluate the efficiency of combined periodontal and triple therapy for H.pylori eradication in patients with periodontitis and peptic ulcer.Methods Forty-three patients with periodontitis and peptic ulcer,were randomized into two groups,experimental group received combined periodontal and triple therapy and the control group underwent only triple therapy.At baseline,3 months and 6 months after treatment,the gastric and oral H.pylori eradication were determined by PCR and compared between two groups.Results The eradication of gastric H.pylori in the experimental group was significantly higher than that in the control group 6 months after therapy（P ＜ 0.05）.The eradication of oral H.pylori in the experimental group was significantly higher than that in the control group 3 and 6 months after therapy （P ＜ 0.05）.The probing depth,attachment level in the experimental group after therapy improved （P ＜ 0.05）.Conclusion In patients with periodontitis and peptic ulcer,the periodontal treatment in combination with triple therapy could increase the eradication of Helicobacter pylori.
