大剂量丙种球蛋白治疗特发性血小板减少性紫癜

目的: 探讨大剂量丙种球蛋白静脉滴注(HDIVIG)治疗特发性血小板减少性紫癜(ITP)的疗效和机制。方法: 对48例中度以上ITP病例按入院顺序分为治疗组和对照组,均给予止血药及肾上腺皮质激素,治疗组IVIG0.4g·kg-1·d-1,连用5天。结果: 治疗组治疗后第5天有76.19%的患者血小板数升至正常,血小板计数回升速度治疗组明显快于对照组(P<0.01)。结论: 大剂量丙种球蛋白静脉滴注治疗ITP,血小板回升快,明显缩短住院日,联合使用肾上腺皮质激素可减少复发。

Effects of high does intravenous immunoglobulin for treatment of idiopathic thrombopenic purpura

Objective: To study the efficacy and mechanism of high does intravenous immunoglobulin(HDIVIG) in therapeutic of idiopathic thrombopenic purpura(ITP).Methods: Forty eight cases of ITP were divided into therapeutic group and control group according to the time of admission to the hospital.They were all given hemostatic and adrenal cortex hormone.The therapeutic group was administered HDIVIG 0.4 g·kg-1·d-1 for five days continously.Results: The number of blood platelet of 76.19% cases in the treatment group rose to the normal level on the 5th day of therapeutic.The Blood platelet count of the therapeutic group rose more rapidly comparing with the control group(P<0.01).Conclusions: Treatment of ITP with HDIVIG can raise the number of blood platelet quickly and shorten the hospital stay.Joint usage of adrenal contex hormone can reduce recurrence.
