螺内酯治疗慢性心力衰竭的临床观察

目的观察常规药物加用螺内酯治疗慢性心力衰竭(CHF)的临床疗效。方法40例心功能Ⅲ~Ⅳ级的CHF患者随机分为治疗组和对照组,各20例。对照组采用常规药物治疗,治疗组则在此基础上加用螺内酯。治疗18周后,观察两组的心功能改善情况以及左室舒张末期内径、左室射血分数的变化。结果治疗组的总有效率为95.0%,对照组为65.0%,两组比较,有显著性差异(P<0.05)。治疗后,治疗组的左室舒张末期内径、左室射血分数较对照组有明显变化(P<0.05)。结论常规药物加用螺内酯治疗CHF的疗效显著,螺内酯在CHF的治疗中有重要意义。

Therapeutic effect of spironolactone on chronic heart failure

Objective To observe the curative effect of combination of routine medications and spironolactone on chronic heart failure (CHF).Methods Forty patients with CHF,whose cardiac function levels were between Ⅲ and Ⅳ,were randomized into therapy group and control group,with twenty patients in each group.The patients in control group were treated with routine medications,and the other patients were treated with combination of routine medications and spironolactone.After eighteen weeks' treatment,the changes of cardiac function,inner diameter of left ventricle at the end of diastole,and left ventricular ejection fraction (LVEF) of all the patients were observed.Results The overall efficacy rate of therapy group was 95.0%,much higher than that of comparison group (65.0%,P<0.05).After treatment,the difference of inner diameter of left ventricle at the end of diastole and LVEF of therapy group were significant,compared with those of comparison group (P<0.05).Conclusion The therapeutic effect of combination of routine medications and spironolactone on CHF is good,spironolactone plays an important role in the therapy of CHF.
