肾小球滤过率评估方程在慢性肾脏病患者中的应用评价

摘    要:目的:评价11种常用的肾小球滤过率(GFR)评估方程在慢性肾脏病(CKD)患者中的适用性.方法: 选择119例CKD患者,用Coekcroft-Gault(CG)方程、MDRD1方程、MDRD3方程、简化MDRD方程、Jelliffe1973(JE73)方程、Mawer(MA)方程、Hull(HU)方程、Jellife1971(JE71)方程、血肌酐倒数公式、Gate(GA)方程、Bjornsson(BJ)方程,分别预测GFR值,与99mTc DTPA测的GFR(sGFR)进行比较.结果:上述11种方程预测的GFR值和sGFR值比较显示,CG方程、MDRD1方程、JE73方程、MA方程、HU方程、JE71方程、GA方程、BJ方程预测的GFR值和sGFR值的差异没有统计学意义.将患者按CKD的不同分期分组,用配对t检验,分别比较上述8种方程预测的GFB值和sGFR值的差异,结果显示只有JE73方程在CKD各期中预测的GFR值和8GFR值的差异均没有统计学差异.结论:JE73方程可能更适合我国CKD患者.

Abstract:Objective To evaluate the applicability of 11 common formulae for glomerular filtration rate(GFR)in Chinese patients with chronic kidney disease(CKD).Methods The values of GFR were estimated in 119 CKD patients using the Cockcroft-Gault equation,MDRD1 equation,MDRD3 equation,abbreviated MDRD equation,Jellife1973 equation,Mawer equation,Hull equation,Jellife1971 equation,reciprocal plasma creatinine equation,Gate equation,and Bjornsson equation.The estimated GFRs were then compared with sGFRs measured by 99mTc-DTPA-GFR.Results No significant differences between sGFRs and GFRs estimated by Cockcroft-Gault equation,MDRD1 equation,Jellife1973 equation,Mawer equation,Hull equation,Jellife1971 equation,Gate equation,and Bjornsson equation were found.However,only the GFRs predicted by Jellife1973 equation had no significant differences from sGFRs at different stages of CKD.Conclusion Jellife1973 equation may be more accurate in estimating GFR in Chinese CKD patients.
