严重多发伤31例救治的分析和研究

目的通过对31例严重多发伤的诊断治疗及伤情评估和死亡原因的分析研究,探索诊断治疗中的关键问题及影响治愈率,死亡率的重要因素。方法本组31例全部有严重胸部外伤,其中开放性胸部伤8例,全部合并血气胸(其中肺裂伤6 例,心脏大血管损伤2例),闭合性胸部外伤合并血气胸23例(其中合并多发性肋骨骨折17例,连枷胸6例,合并肺挫伤6例);6 例合并严重的腹部外伤,其中脾破裂3例,肝破裂2例,肾挫伤2例;3例合并严重的颅脑外伤;21例合并创伤休克。结果治愈19例(占61.29%),伤残5例(占16.13%),死亡7例,病死率为(22.58%)。结论及时准确诊断是降低严重多发伤死亡率的首要条件。合理掌握优先处理的顺序,按照抢救-诊断-治疗-复诊-再治疗的步骤进行,正确掌握手术顺序是抢救成功降低死亡率的关键,及时处理胸壁软化所致的连枷胸,早期气管插管机械通气是提高治愈率的重要因素。
