益气滋阴活血化瘀法对糖尿病肾病的疗效分析

目的 探讨益气滋阴活血化瘀法治疗糖尿病肾病（DN）的疗效及对血液流变学、氧化应激的影响。方法 选取2015 年10 月—2017 年10 月唐山市中医医院诊治的110 例DN 患者，采用随机数字表法分为对照组（采用缬沙坦治疗）和观察组（采用益气滋阴活血化瘀法联合缬沙坦治疗），每组55 例。于治疗前后行肾功能、血液流变学及氧化应激指标检测，比较两组患者临床疗效及安全性。结果 观察组患者治疗前后内生肌酐清除率、24 h 尿蛋白排泄率、尿素氮、血肌酐、全血黏度低切、全血黏度高切、纤维蛋白原、红细胞沉降率、超氧化物歧化酶、丙二醛及总氧化能力的差值高于对照组（P <0.05）。观察组总有效率高于对照组（P <0.05）。两组不良反应（肝功异常、一过性低血压及胃肠道反应）发生率比较，差异无统计学意义（P >0.05）。结论 益气滋阴活血化瘀法联合缬沙坦治疗DN 的疗效显著，可改善血液流变学状况，减轻氧化应激反应。

Application effect of Yiqi Ziyin Huoxue Huayu methodon diabetic nephropathy

Objective To investigate effect of Yiqi Ziyin Huoxue Huayu method combined with valsartan ontreatment for diabetic nephropathy and its influence on hemorheology and oxidative stress. Methods Totally 110patients with diabetic nephropathy were selected in hospital from October 2015 to October 2017, who were dividedinto two groups by random number table method. 55 patients treated by valsartan as control group. 61 patients treatedby Yiqi Ziyin Huoxue Huayu method combined with valsartan as observation group. Indexes of renal function,hemorheology and oxidative stress were detected before and after treatment. Clinical effect and safety were comparedbetween two groups. Results Differences of Ccr, 24 hUab, BUN, SCr, NBL, NBH, Fb, ESR, SOD, MDA and TAOCbefore and after treatment in observation group were higher than those in control group(P < 0.05). Total effectiverate in observation group was higher than that in control group(P < 0.05).There were no significant differences inadverse reactions (abnormal liver function, transient hypotension, gastrointestinal reaction) between two groups(P > 0.05). Conclusions Yiqi Ziyin Huoxue Huayu method combined with valsartan have significant effect on treatment for diabetic nephropathy, which can improve hemorheology and relieve oxidative stress.
