枸橼酸咖啡因联合机械通气治疗新生儿呼吸窘迫综合征的疗效

摘    要:目的 探讨机械通气联合枸橼酸咖啡因治疗新生儿呼吸窘迫综合征(RDS)的临床疗效。方法 选取2014年5月—2017年6月安徽医科大学第二附属医院新生儿重症监护病房收治确诊为新生儿RDS且需要机械通气的患儿100例,在入院后按照就诊时间随机分成枸橼酸咖啡因+机械通气治疗组(联合组)以及机械通气治疗组(对照组)。比较两组间的机械通气时间、给氧时间、住院时间及相关并发症发生率等指标。结果 联合组的患儿平均机械通气时间、平均给氧时间均低于对照组,差异有统计学意义(P?<0.05)。但平均住院时间和院内感染发生率,两组差异无统计学意义(P?>0.05)。结论 新生RDS患儿给予枸橼酸咖啡因治疗可有效缩短机械通气时间和给氧时间,但在缩短住院时间及降低院内感染发生率方面无优势。

Abstract:Objective To investigate the clinical efficacy of caffeine citrate combined with mechanical ventilation therapy in neonatal respiratory distress syndrome (RDS). Methods Totally 100 newborns diagnosed with RDS and requiring mechanical ventilation were enrolled in NICU of our hospital from May 2014 to June 2017. They were randomly divided into the Caffeine citrate + mechanical ventilation treatment group (combination group) and mechanical ventilation treatment group (control group) according to the time of treatment after admission. The indexes of mechanical ventilation time, oxygenation time, hospitalization time and related complication rate were compared between the two groups. Results The average mechanical ventilation time of the combination group was lower than that of the control group, and the difference was statistically significant (P??0.05). Conclusions The treatment of Caffeine citrate in newborns with RDS can effectively shorten the time of mechanical ventilation and oxygenation, but it has no advantage in shortening the length of hospital stay and reducing the incidence of nosocomial infection.
