目地 探讨和评估PFNA-II联合外侧支撑钢板内固定重建治疗外外侧壁不稳定型股骨粗隆骨折的治疗体会。方法 2012年1月-2016年12月,通过切开复位,应用PFNA-II联合外侧支撑钢板治疗股骨粗隆并外侧壁不稳定型骨折 21例,通过术后随防复查评估骨折复位质量及愈合情况,骨折愈合后按Harris髋部评分系统评价患者的功能情况。结果 本组21例患者通过支撑钢板复位固定股骨粗隆外侧壁骨折和PFNA-II髓内固定后均获得了良好的复位、固定牢固。所有患者均获得随防,随防时间3-18个月,平均(9+2.6)个月,所有患者均获得骨性愈合。术后根据Harris功能评分：优16 例,良4例,可1例,差0例,优良率95.2%。结论 通过PFNA-II联合外侧支撑钢板是治疗股骨粗隆并外侧壁不稳定型骨折的有关效方式,保留了股骨粗隆外侧壁的完整性,增强了骨折端的稳定,具有较强的坚固性和稳定性,提高了固定疗效。

Objective Discussion and evaluation PFNA reconstruction - II joint lateral supporting plate fixation treatment of outer wall is not stable in the treatment of femoral trochanter fracture. Methods In January 2012 - December 2016, by open reduction, the application of combined lateral PFNA - II support plate for the treatment of unstable lateral femoral trochanter and wall type 21 cases of fracture, through quality and assessed with the review after fracture healing, according to Harris hip score after fracture healing system evaluation function of patients. Results The 21 patients in this group received a good reposition and fixed strength by supporting the fixation of the lateral wall fracture of the femur and the pfna-ii pulp. All patients received the following protection, with a period of 3-18 months, averaging (9 + 2.6) months, and all patients received bone healing. According to the function of Harris, there were 16 cases, 4 cases, 1 case, 0, and 95.2%. Conclusion Through the PFNA - II joint lateral supporting plate is the treatment of unstable lateral femoral trochanter and wall type fracture mode of the effect, maintain the integrity of the lateral femoral trochanter wall, enhance the stable fracture end, has strong robustness and stability, and improve the fixed effect.
