胃转流术治疗2型糖尿病围手术期护理

目的:探讨2型糖尿病患者行胃转流术围术期的护理方法。方法:对11例2型糖尿病行胃转流术患者实施精心的围手术期护理,包括:术前做好护理评估,心理护理,严格控制血糖,指导合理膳食;术后注意病情观察、血糖监测和控制,加强营养管护理,提供合理的营养支持,观察、预防并发症的发生,并注重出院指导及随访。结果:11例患者术后1个月血糖均有明显下降,葡萄糖耐量明显增强(P0.01),3例术后随访1年,血糖在正常范围不需药物控制;糖尿病相关并发症均有不同程度缓解。有3例患者术后并发胃排空障碍,经保守治疗痊愈。结论:胃转流术能有效治疗2型糖尿病,手术安全可行。对2型糖尿病行胃转流术患者实施精心的围术期护理,可确保手术顺利进行,减少术后并发症发生。

Perioperation nursing of gastric bypass treatment in patients with type 2 diabetes mellitus

Objective：To investigate the nursing of Roux-en-Y gastric bypass in type 2 diabetes mellitus（T2DM）. Methods：Clinical data of 11 patients with T2DM treating by Roux-en-Y gastric bypass were retrospectively analyzed.We finished preoperative assessment,and paid attention to the state of illness and the mental nursing to avoid complication.Results：The blood glucose was decreased obviously after one month in 11 casee.Gloucose tolerance enhanced（P〈0.01）. Complications diabetes-related relieved,too.3 cases was complicated by delayed gastric emptying after operation and cured by conservative treatment. Conclusion：Roux-en-Y gastric bypass could treat T2DM effective,and safely. It was strict in postoperative nursing.So as to let patients pass perioperative period safely,taking effective nursing during perioperative period was of great importance to patients postoperative healing,and it was the linchpin to enhance the security of operation and avoid compliCation.
